Overview

The Prescription Refill Nurse is a licensed nurse (RN or LVN) responsible for efficiently and accurately managing medication refill requests for patients within the Federally Qualified Health Center (FQHC) setting. This role ensures adherence to established clinical protocols, regulatory requirements, and FQHC guidelines, while also providing patient education and supporting smooth transitions of care. 

Qualifications

  • Diploma or Associate's Degree in Nursing from an accredited program (for LVN/LPN), or Bachelor of Science in Nursing (BSN) from an accredited program (for RN).
  • Current and unrestricted Registered Nurse (RN) license or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license in the state of Georgia.
  • 1-2 years of nursing experience, preferably in an outpatient, ambulatory care, or community health center setting.
  • Experience with electronic health record (EHR) systems (e.g., athenaNet, Epic) and medication management software is preferred.
  • Strong understanding of pharmacology and medication management principles.
  • Cultural sensitivity and ability to effectively communicate with diverse patient populations.
  • Current CPR certification (BCLS by AHA).
  • Knowledge of medical procedures, which relates to patient education and health practice.
  • Excellent communication, organizational and customer service skills.
  • Excellent phone etiquette.

Responsibilities

  • Review and process prescription refill requests received via phone, patient portal, fax, and other channels.
  • Assess patient records for medication appropriateness, verifying prescription details, dosages, and compliance with provider orders.
  • Route requests to the appropriate prescribing provider for authorization or consultation, collaborating closely with providers and other healthcare team members.
  • Identify and triage urgent refill situations, escalating to providers as needed.
  • Provide patient education regarding medication purpose, administration instructions, potential side effects, and adherence to treatment plans.
  • Respond to patient medication-related questions and concerns within the scope of practice.
  • Monitor for potential drug interactions or contraindications, escalating concerns to providers and pharmacists.
  • Coordinate with pharmacies to ensure timely and accurate dispensing of medications, resolving issues as needed.
  • Accurately document all medication refill requests, actions taken, and patient communications in the electronic health record (EHR) system.
  • Ensure compliance with Federally Qualified Health Center (FQHC) guidelines, federal and state regulations, and internal policies related to medication management and patient confidentiality Health Insurance Portability and Accountability Act, (HIPAA).
  • Participate in quality improvement initiatives related to medication management and patient safety.
  • Collaborate effectively with providers, pharmacists, other nursing staff, and administrative personnel to ensure seamless medication management and patient care coordination.
  • Communicate relevant patient information and updates to the primary prescribing provider and other team members.
  • Other duties as may be assigned.
