High school diploma or equivalent
Knowledge of local service networks and external systems relevant to school‑based care coordination
Ability to maintain and develop working relationships with organizations that provide resources to students
Skill in identifying contacts within organizations and establishing communication pathways that support coordination activities
Ability to communicate directly with students of various ages in a clear and professional manner
Experience interacting with a range of student groups and service organizations
Use clear messaging to support enrollment and service use within school‑based health programs
Ability to function as part of a team and address conflicts when they arise
Experience navigating multiple systems including health, education, and administrative processes
Strong time‑management skills with the ability to balance competing tasks
Problem‑solving abilities and adaptability in a fast‑paced environment
Ability to maintain an updated resource inventory and identify appropriate options based on student needs
Work with students and designated caregivers along with school health personnel to carry out written care plans that include health summaries and action steps.
Coordinate scheduling, referrals, and follow‑up activities to support care‑management processes.
Track completion of assessments and ordered tests and provide updates to the clinical team as needed.
Guide students and designated caregivers through required processes across health, educational, and administrative systems, including consents, privacy protocols, and insurance steps.
Use health information systems to monitor care‑coordination tasks and ensure timely follow‑through.
Assist with data collection and documentation using electronic records or other approved tools.
Communicate effectively with students and designated caregivers to ensure clarity around care‑coordination expectations.
Maintain working relationships with school personnel and clinical teams involved in school‑based health services.
Participate in school‑based activities that increase awareness and appropriate use of school‑based health services.
Share information appropriately with students, designated caregivers, school staff, and other approved parties in alignment with permissions and privacy requirements.
Provide information on available health and service resources including primary care, behavioral health, vision, oral health, transportation, and financial‑assistance options.
Assist designated caregivers with required forms and documentation such as consents, insurance paperwork, reimbursements, and referrals.
Support compliance with documentation standards for school‑based care‑coordination activities.
